CAPS Pet Shop & Online Seller Complaint Form
In order for CAPS to be able to follow up on a pet shop complaint, we will need you to first sign up for an account, then access your account via the Login link. Once you have registered and logged in, the Submit complaint link will be accessible from this page.
When you register for an account on our website, you can:
